Student Transfer Between Schools Form

If you hold the Ontario Student Record (OSR) for a new student, use this form to transfer their OUAC account to your school.

Provide all the following information for the student who is transferring to your school. The OUAC will process the student transfer once we verify the information you provide in this form with the previous high school.

If the student has transferred from a school within your board, that school can process the transfer themselves. They must:

  • log in to COLS,
  • search for the student in Academic Information,
  • change the MIDENT in the Diploma Details tab and
  • save the change.
